Diabetes & Weight Loss Center is your premier center for diabetes and weight loss program. Our practice was founded by Dr. Adriel Perez, DNP! Adriel is an Autonomous Advanced Practice Registered Nurse (A-APRN) and Certified Family Nurse Practitioner (FNP) who is passionate about providing patients with the tools they need to carry out their best lives. As an autonomous FNP, Adriel provides primary care services with a focus on diabetes and weight loss program. Our ultimate goal at Diabetes & Weight Loss Center is to ensure our patients lead a healthy life that suits their lifestyle. We don’t want our patients to compromise who they are for their diabetes or weight loss program – rather, we want them to feel in control of their conditions and live their lives to the fullest. We are proud of the services we offer to our patients, in both English and Spanish!
